Important Information
  • Amendments: all revisions, additions or deletions to approved studies
  • All amendments must be submitted via CAPCR for review to ensure that the research remains scientifically and ethically sound.
  • If your submission requires a Full Board review, please consult the submission deadline dates found here to determine the appropriate date.
  • The Principal Investigator (PI) is responsible for ensuring that amendments are submitted to the REB for review.
  • REB approval must be received prior to implementation.
Types of Amendments
Form Amendment Administrative Change Change in PI
Description A substantive change to the design or conduct of a study Minor changes to the study that impact REB-reviewed Study Documents Changes to the Principal Investigator for a study
 
Making a Change
  1. Complete the amendment via CAPCR
  2. Include all documents affected by the amendment
    • Revised Protocol (tracked and clean copies), Informed Consent Form(s), IB, etc.
    • Supporting documents (i.e. new information supporting amendment)
  3. Ensure version dates are updated for documents that are updated
  4. Retain a copy for your records.
