Psilocybin-assisted Existential, Attachment, and RelationaL (PEARL) Therapy for Patients with Advanced Cancer: A Phase II Open-Label Trial

Study Purpose: This study aims to determine whether a new treatment (medication-assisted psychotherapy) can be given, and is seen as useful, to patients with advanced cancer. This new treatment is called Psilocybin-assisted Existential, Attachment, and RelationaL (PEARL) Therapy.
Background: Patients with advanced cancer experience many mental health challenges including complex treatment decisions, changes in identity and close relationships, loss of meaning, and fears for the future. Emerging evidence shows that psilocybin-assisted psychotherapy (ingesting psilocybin in conjunction with psychotherapy) may reduce end-of-life distress for cancer patients. Our team has developed a novel intervention called PEARL therapy, created using evidence-based psychotherapeutic methods that are designed for those with advanced illness. PEARL was developed to help people manage the challenges of living with cancer, reduce distress, and promote well-being.
Study Methods: The study involves 8 psychotherapy sessions delivered over 2 months. The sessions are conducted by specially-trained therapists who will get to know the patient and their history. One of the sessions will be an 8-hour psilocybin session. The patient will ingest a pill containing psilocybin. In sessions before and after taking the psilocybin, therapists will support patients regarding their own unique individual challenges and situation by exploring: disease and symptom management, changes in personal relationships, spiritual well-being, or maintaining a sense of meaning and purpose, preparing for the future and facing mortality. Study participants will complete interviews and questionnaires to evaluate their experience and changes in their level of distress.
