How to Renew a Protocol

In accordance with REB guidelines, approved studies must be renewed annually.

The Annual Review/Termination Report Form must be submitted to the REB prior to the expiration date for approved projects.

The Research Ethics office requires hard copies of all applications with original signatures. Electronic, photocopied or faxed submissions will not be accepted.

Completed Annual Review/Termination Report Forms for Oncology, Multidisciplinary /Non-Oncology, and for Human Tissue Committee studies should be submitted to:
